Gabe’s Chemo Duck Program always provides Chemo Ducks free to families. Our goal is to have Chemo Ducks available upon diagnosis through your local hospital. However some clinics and hospitals are not yet aware of our program.

Please help us introduce Chemo Duck to your hospital. Print the forms below and take them to your child life specialist, they can place an order for a free Chemo Duck for you. If you are interested in helping us to raise awareness of the need for Chemo Ducks at your facility please contact info@chemoduck.org.
If you prefer not to go through the process of trying to request a Chemo Duck for free through your hospital, you have an option of ordering a duck directly.
